Problems with Form 1099-MISC

I received a Form 1099-MISC from an employer.
We had some complications and i quit.
They sent me this form with just the Payers name, the Payers federal ID number, my name not correctly written , and an amount in Nonemployee compensation.

so would i have to file this form?
