What are the top reasons for a delay in my tax refund?

The IRS has stated that there are a "variety of reasons" that causes a delay in the tax refund especially for a paper filed tax return. The IRS has cited the following as the most common reasons responsible for the delay in the tax refunds as follows:

1. A name and social security number listed on the tax return may not match the IRS records. This would not only include the taxpayer and the spouse's name and SSN, but also that of their dependents name and SSN.

2. A taxpayer (or a spouse for a jointly filed tax return), has failed to sign or date the tax return.

3. A taxpayer has made a mathematical error that would require extra time for the IRS to correct.

4. A taxpayer has failed to include a necessary attachment, such as the W-2's for a paper filed return.

5. The tax return has not been properly submitted in the proper sequence as required by the IRS.
