Hello, I am having a bit of trouble locating the proper forms.

I created an LLC through Legal Zoom in late 2010 with not much income. It was primarily created to prepare for business in 2011, but did take income in 2010.

I am using taxslayer.com to file. Legal Zoom states I need a form D-403. I can not find this forum at taxslayer.com

Has anyone experienced this before? and is this the same as the schedule C area of taxslayer.com?

Basically I filled out the schedule C area perfectly, but I see no mention of this ford D-403 and wonder if this schedule C area is good enough.
