IRA Beneficiary Distribution Form

I received a IRA Beneficiary Distribution Election Form and I am not quite sure what to do. Am I supposed to withhold 10% and my state tax? What happens if I don't take any taxes out? Can I roll over the account to another IRA?

If anyone could gie me some advice, that would be great.
