Smile Local Withholding & Localities & Disability

Hello,
I file my taxes myself every year with TaxAct, but I always have two issues. I've uploaded some screenshots, too.

First, I always get the error every year that the "local withholding has been reported to be greater than 50% of the local wages of federal wages." I am typing down every number exactly as it appears on my W2 form. I usually hit continue, every year, and I don't think it's an issue, but I want to make sure everything is totally accurate, this year. What's wrong there?

Next, I have two different amounts, with two different codes in each row. How do I put those both in? Do I add them up & use "other locality" for the code names?

Lastly, I am not sure exactly what disabilities I have to put down on my return. I have bad vision, but I'm not legally blind. Also, I have been diagnosed with depression & anxiety, and I have been prescribed medication for it.
