No 1095 form

i applied for health coverage in 2015 with United Health Care, but i never made any payments so after three months they canceled my coverage, and the irs requires me to have a 1095a form for filing this year but on my market place account theres no 2015 tab for anything relating to being covered in 2015, but im being asked to provide that form that i cant file, ive called the irs, they said to call marketplace, so i called the marketplace and they told me to contact united health care, so i contacted them and they told me to contact the market place that i could get the form from them, idk what to do at this point and the filing deadline is right around the corner and i need to get my tax refund, what do i do

as you fail to pay your premiums and exhaust the grace period for plans offered in a health insurance marketplace, you will lose your insurance coverage. The grace period is either one month or three months long, depending on whether or not you?re receiving subsidies and whether or not you?ve paid at least one premium so far during the year.; The 3month grace period applies to those who receive federal subsidy assistance in the form of an advanced premium tax credit and who have paid at least 1full month?s premium within the benefit year. For those without a subsidy the grace period is 1 month (the 1 month grace period also applies to plans purchased outside the exchange, since none of those plans qualify for subsidies). it?s common for people to receive an incorrect Form 1095-A if their coverage was cancelled retroactively due to non-payment of premiums; in that case, the exchange will issue a corrected Form 1095-A. If in doubt, contact the exchange and/or your tax adviser

So how do i go about actually getting the corrected 1095a form, i have not received any form prior yet even. And who is the "exchange" and how do i contact them, All i need is a 1095a form for the year 2015 for the little grace period. those few months i was active in coverage but didnt even make payments thus cancaceld the membership.

So where do i actually get the 1095a form from, that i have not yet recieved for the 2015 coverage i was enrolled in, active, but never made a payment thus canceled my membership. Who is the exchange? how do i contact them, all i need is a 1095a form for the 2015 premium i had for feb, march,april. which isnt on my marktetplace account, or to be found anywhere. i still dont get what to do or how to obtain this information

who are the "exchange" i dont queit understand, i need a 1095a form for the 2015 coverage i only had for 3 months because i exhausted my grace period. I cannot find the 1095a form on my maketpalce account. The marketplace only advises me to contact the insurance company for that information. and then they tell me that the marketplace will have it. my only issue is is that i need a 1095a form for the year 2015, and i cannot find the 1095a form

missing 1095a

I dont queit undestand, all i need is my 1095a form for the 2015 year i had coverage for 3 months on because i exhausted my grace period and ths it got canceled, either way. I had that premium in 2015, so i need a 1095a form for that, Which i cannot find in marketplace account, so i ask marketplace and they tell me to contact that insurance company, so i do, and they said no no it should be in marketplace. where and how exactly do i obtain my 1095a form if the marketplace account cant even provide that for me.
