What tax form is required to claim the tuition deduction and fee expenses in 2009?

The IRS has required the use of Form 8917 for taxpayers to determine the deduction for tuition and fees expenses paid in 2009. The IRS has determined that "this deduction is based on qualified education expenses paid to an eligible postsecondary educational institution."
