Where should taxpayers file Form's 8804, 8805 and 8813?

Per the IRS instructions, taxpayers are required to file Form 8804, 8805 and 8813 to the following addresses as shown below:

Internal Revenue Service Center
P.O. Box 409101
Ogden, UT 84409
