Form 8606

I need to fill out a form 8606 for my 2008 taxes. I didn't know that I needed to fill out the form at the time I filed. My husband had a mandatory rollover from his employer from a Simple to a Roth. We later that same year cashed out on the Roth.

Line 16 = The amount of the Rollover from a Simple to a Roth?
Line 17 = How do I figure this amount?

Line 19 = The amount of the cash we received from the distribution from the Roth above

Line 22 = The amount that the Roth was worth at the time of the distribution?

Line 24 = The amount that was rolled from the Simple to a Roth, from the first transaction in part 2 of the form?
