Self-employed and moved jobs in the middle of year

I am self-employed and moved jobs in the last couple months of the year 2015. I made some calculation errors on my taxes, and the IRS is requesting I fill out another schedule c form.
Half of my deductions are from the first job, and the adjusted deductions are from the second place.

My question is, when it asks for the business address which address should I choose? The last place, or is there a way to put both?

Additionally, when it asks did I start or acquire this business in 2015, should I say yes?

Thank you for any help, I?m obviously new at this.

The last place, or is there a way to put both? ==========> When you're running two businesses with two different addresses that are taxed as sole proprietorships, the IRS requires you to report the income and expenses of each business separately.So you need to report each address on each separate Sch C.

Additionally, when it asks did I start or acquire this business in 2015, should I say yes?=========>>it depends;aslongas you acquired the second biz in 2015 as a new owner of the second biz , then yes.
