ACA and choosing not to disclose

Hello. If I choose not to disclose my healthcare status on my tax return and the IRS has the 1095 form already, will that trigger a response from the IRS?

We were covered part of the year, and that causes us to pay a penalty. We could barely afford the premiums let alone penalties.

What do you think?
