Need some W-2 clarification

I received my W-2 from my employer, it was 2 pages. Regarding the federal portion, the first page appears as normal with boxes 1-6, and 12a and 12b filled out. Box 15 lists NJ as the state. The rest of the line across (boxes 16-18) are blank, except Box 19 but the number is below the dotted line (why?). It is also a very low number.

The 2nd page has nothing in boxes 1-10,12, etc. It has "********" in them. Box 15 lists PA as the state and lists state wages, tax, etc (Boxes 16-20 all filled out).

I've had the same employer all year, but I'm a consultant, so I worked on a short project in the beginning of the year in NJ and the rest in PA. I live in PA.

I'm not sure what to make of this and how to file my Federal. Also do I have to file state returns in both NJ and PA?
