Unhappy Employee cash value life insurance paid for by 'er

My husband recieved both a W2 and a 1099 (non-employee compensation) from his employer. The 1099 is for the life insurance premium his employer paid where I am the listed beneficiary. This policy was given to him when he inquired about a retirement investment plan for his future.

Is this correct that he should have recieved both a W2 and 1099.
Do we have to pay taxes on this policy premium?
