According to the Maryland Department of Revenue, you can mail all of your Maryland income tax forms and payments to:

Comptroller of Maryland,
Revenue Administration Division,
Annapolis, Maryland 21411-0001

A copy of the Pages 1 throught pages 5 of the Federal Income tax return, Form 1120 must be attached to the Maryland Corporate tax return, Form 500.
