Tax Levy California FTB and Hardship

Hello everyone, On Thursday of last week the California FTP placed a hold on my bank accounts. To be honest, I had no idea what happened and the money that was in the account was literally taken (as they do) without prior notice.

Come to find out they had been sending notices, but, had the wrong address and wrong spelling for my last name. Regardless, I was still levied and now am up against something I really don't understand.

They want me to fill out a hardship form, which I have no problem doing, but the matter is complicated because of the fact that I own my own business and sometimes get large amounts of cash that don't reflect income, but, payment for equipment, etc. The money that was levied was just that - money for equipment for a customer.

So, I guess my question is, what is the best way to present my case- I live in AZ now and all they want is my bank statements and the form but I don't believe it truly represents what is going on in my financial situation. Does anyone have any advice for me? I was told that I will most likely have the monies release but I want to provide as clear of a picture as possible for them to understand what type of a situation this has put me in, not being able to purchase the goods for my clients, not being able to pay my monthly bills and mostly the true hardship this has placed on me, my family and my business.

Regardless, I was still levied and now am up against something I really don't understand.=====> What doesn't make sense at all is that you didn't receive a whole series of notices, letters etc., before this ever happened. But it is your responsibility to report any Federal changes to California.
