1099r

I received a 1099R from my employer for my pension for 2014. I retired in December 2013. Boxes 1 & 2a both have the same dollar amount. No Federal income tax was withheld. Box 7 has a distribution code of "2". I just turned 60 years old in November of 2014. How, or do I need to fill out form "5329"?
