Unhappy voluntary adjustments ?

Hi All,

I got voluntary adjustments as follows:

VOLUNARY ADJ YTD AMOUNT FED WAGES CALIFORNIA WAGES

MEDICAL 2644.48 - 2644.48 -2644.48

PER DIEM 3000.00 N/A N/A

Can any one explain what does the above information means and can i claim the medical insurance amount that paid ( 2644.48 , I paid 75% of medical insurance and i took employer provided insurance)

I appreciate all the help.
