Capital Gains/Loss selling of bank stock

I had a few shares of bank stock. A bank sale took place at the end of 2012. the bank stock was liquidated in Jan. 2013. The agreement was there was a cash consideration and a note consideration for the total price of the stock. the cash portion was paid in January for an agreed upon price. The Note portion was to be paid out over 4 years depending on the quality of the loans that were purchased by the new bank. Should I have a 2013 Form 1099-B for the cash sale?
