What is an " Item" claimed on Form 8283?

My question concerns 2009 Charity Donations claimed on Form 8283 for an "ITEM" that is $500 or less but donation total up to $5,000.
1. When using DeductionPro which is linked to TAXCUT software I listed 30 or more items which, in total, exceeded $500. (i.e. if you donate to Purple Heart Veterans 30 separate goods that include clothing items with valued thrift store values, listing Men's and Woman's clothes, kitchen items, bedding & linens which, in the aggregate, total up to an amount more than $500). I was then referred to Form 8283 to complete parts (d)(e)& (f).
So is the IRS definition of "ITEM" actually the sum total of all the goods given to that particular Charity Organization?

So if you tell me $500 is the amount allowed that you can claim without further completion of (d)(e) & (f) then the next question is:
2. I have donated to 3 other different Charity Organizations this year. Am I allowed $500 for each organization for a sum total of $2,000 in Charitable Donations without having to complete (d)(e)&(f) on Form 8283?

3. DeductionPro describes the donation generally (i.e. Men's dress slacks or books-fiction paperback). Do you then need to attach a more descriptive itemization to that? (i.e. name of book: "To Kill a Mocking Bird"- Author: Harper Lee and Ralph Lauren Men's Dress Slacks) or does the DeductionPro list suffice?
