1099-R distribution code

I rolled my 401k from my former employer directly into a Roth IRA. What is the right distribution code? The company that sent me the 1099-R already sent it to wrong once and I still think it is wrong. They have $0.00 listed under the taxable amount which seems wrong to me to. Shouldn't it all be taxable since I am putting it into a Roth IRA?
