Amending a 1139 with one year passed

Tax Guru,

I was hoping you could help. I want to amend a 1139 (where more than 1 year has passed since the tax year in which the carry back occured). I understand that I must use for 1120x. However, I am not sure I am completing the for to correctly amend the 1139. I am trying to adj a 250,000 carryback. Any instructions or thoughts would be appreciated.
