Return of certain funds after 401k rollover

After rolling over my 401k to a traditional IRA, I received a check for $50.00 and a Form 1099-R. Out of my total 401k $50.00 could NOT be rolled over. The form 1099-R shows a Gross Distribution: $50.00; Taxable Amount: $0.00; Box 2a Total Distribution; Box 5: $50.00; Box 4 & 12: $0.00 (no Taxes withheld).

I believe this after tax $50.00 was a start-up fee to establish the original 401k account.

I have never contributed to a ROTH IRA.

How do I include this $50.00 in my Form 1040 Tax Return?
Do I need to file Form 8606?

Any help would be appreciated. Thanks
