Qualifying Widower

Hey guys, Sorry for asking such a basic question but I am really unsure of proper format on this situation below:

Tom's spouse died in 2012, he has maintained a household with a dependent child. Therefore he meets the requirements for qualifying widower. Should I include his spouses Social Security number on his 1040 for 2014 and also should I check both boxes 6a and 6b???
