HSA confusion

Freetaxusa describes An HDHP as s health plan that meets the following requirements:

SELF ONLY COVERAGE:
Minimum annual deductible of $1,250
Maximum out of pocket expense: $6,350

I have an HSA account. Anthem says this 1500 deductible for a person
Max out of pocket 3000 in network
6000 out of network


freetaxusa asks me if i have an hsa....i say yes
Did i recieve a distrubution? NO
Was the coverage under the high deductible health plan for self or family?

Choices are self or family...Is it even a HDHP?

Did employer contribute anything else besides what's already claimed? No

Did you make any contibutions? no

Were you enrolled as of december 1st 2014? yes

I'm really confused if i should be even messing with this form on freetaxusa but i do have an hsa.
