401k withdrawn , shal i claim my hardship expense from tax excemption

i have withdrawn 401k for my wifes medical expenses, as per IRS rules, i can claim the amount which i spend from tax exemption. let us i have spend $5000 for my wifes medical expenses, while filing my tax return do i need to mention this amount for tax exemption, i am filing my returns through online, where there is column for hardship expenses, my question is do i need to submit my wifes medical bills to have hardship expense for tax free? or just mentioning the amount will do?
