Household WOrker - Daughter under 21

I paid my daughter - who is under 21 - a little over $2,000 for childcare of my two younger children in 2011. Since she's my child, and under 21, I understand that the wages are not subject to FICA/FUTA tax. Should I have still given her a W-2?
Also, should this income be reported on a financial aid (FAFSA) application?
