Form 1099 R

Form 1099 R: My client received this form with an amount of $11,417 showing on box 1 and 2a; box 2b has an x (taxable amount not determined) which she stated is incorrect since she had invested her own money of $11,000(5 yrs ago) into an IRA acct which should not be taxable. She never took a deduction on her tax return for this amount. She discussed this matter with her bank but they would not correct this 1099 R.
How should I show this on the tax return?
