Form Help and 401 k help

Hi. here is my question. my father always did my taxes. but, not this year...so, i got fired from my job. i had elected into the 401 K program. but, not for a very long time. thought it was stupid. well, until i got fired and was told i was allowed that money without a penalty for early withdrawal. so, my taxes are usually very easy. i assume that 401k is also considered a SIMPLE plan the 1040 book keeps talking about. the amount i got from the 401k was about 2700 with taxes taken out (not state taxes) about 560 was taken out. left me with about 2200.

1. do i need to file this?
2. what form do i need, or can i just add it to line 38 of the 1040A book i got in mail? Meanwhile, how come no form was mailed to me, which makes me think i dont need to file. do i need a 1099-R form. cause abnother guy you said a 5392 (or something ) form
3. Am i allowed back any of that money they allready took.? can i use that money to my advantage at all?

thanks so much. please explain as if i was 4. like i said....never did my own taxes before. and i am broke. so i need to do it on my own. thanks a lot
