Missed Depreciation on Rental property

HI,

We own a rental property which we first had for personal use and later placed in service for rent in June 2014.

We just realized that we missed claiming depreciation on this property for part of 2014 and entire 2015.

I know we need to file amended return (1040-X) for both years and are eligible since it's been less than 3 years since we put it in service.

My question is do we also need to file Form 3115 with our 2017 tax return? We are in process of preparing 2017 tax return and have not filed yet. We are planning to file before April 15, 2017.

Please help!
