Actually, you can request a copy of your 2004 Federal Tax Return Transcripts from the IRS. The process may take a couple of weeks.

Using the information from your Transcripts you can determine your W-2 Income that you earned in 2004. The State withholding taxes can be determined from contacting the NJ Division of Taxation. I think that the amount of withholding would be very little to none as all your income is sourced to NY State.

You should also contact NY Dept of Taxation to ensure that you have filed your 2004 tax return. Also, see if they can provide a copy of your 2004 transcripts or a copy of that tax return. This way, you can inform NJ Division of Taxation, that your W-2 Income was sourced to NY State and provide them a copy of the 2004 NY Tax return/Transcripts.

Using this information, your Accountant/Tax Preparer can quickly prepare your 2004 tax return. Also, I do not think that you would be subject to any substantial tax liabilities for NJ State as all the income earned is NY sourced, and thus this would not result in any material NJ Tax Liability!
